2. HCM case study A5 booklet (87K)

This document is intended for use as an A5 booklet. It comprises a landscape page - half-page for the A5 back page with an introduction to the HCM. Then a front cover with explanation. The final page in (portrait) format the booklet's centre has a case study HCM map and rationale. Our hope is that visitors will use this format to create a series of HCM guides covering several fields of nursing, medical and health care specialities. The potential could be further extended using the XML document standard.

This is copied from APPENDIX 4 of Blackburn I-M, Davidson K (1995) Cognitive Therapy for Depression and Anxiety, Blackwell Science. Photocopying the page - as the authors seem to have intended - results in much wasted space, in addition to stressing the spine. The book is well worth purchasing. Word 97 file.
